The Wisconsin Department of Revenue (DOR) received gross patient revenue data from all of the ASCs in Wisconsin subject to the assessment in 2012, and has calculated the assessment rate. The 2012 gross patient revenue reported by all ASCs totals $418,489,270.00, resulting in an ASC assessment rate for 2012 of 3.9905%.
