Paper Claims Received by ForwardHealth on and after January 1, 2012, Must Be 5010/D.0 Compliant

Paper claims received by ForwardHealth on and after January 1, 2012, must be completed using the revised claim form completion instructions and/or revised claim forms, as appropriate, or they will be denied, as a result of implementation of HIPAA version 5010 and NCPDP version D.0.
This impacts 1500 and UB-04 claim form completion instructions and Compound and Noncompound Drug claim form completion instructions and forms.
Refer to the Claims section of the Online Handbook on the Portal for revised 1500 and UB-04 completion instructions and sample claim forms.
The D.0-compliant Noncompound Drug Claim, F-13072 (09/11), Compound Drug Claim, F-13073 (09/11), and completion instructions are on the Forms page of the Portal.
